E-Posters

E-posters will be presented and displayed in the Exhibit Hall booth #103 located in Pre-Conference A. E-poster presentations will take place on September 18 and 19, during lunch, as per the schedule below. Presenting authors are expected to be at booth # 103 during the specified presentation time to facilitate discussion and answer questions. E-poster presentations will be 15 minutes in length and take place consecutively.

**E-posters will be rotating on the poster screen during regular conference hours when poster presentations are not taking place.

E-POSTER #1 - Project ECHO Care of the Elderly-Long-Term Care

This poster will introduce ECHO Care of the Elderly-Long-Term Care, an educational program that uses videoconferencing technology to build capacity of primary care providers and interprofessional team members while promoting multi-dimensional learning.  A live mini “mock” ECHO session will be utilized to demonstrate the program.

E-POSTER #2 - Benefits of Implementing the OLTCA Spasticity Management Program in Ontario Long Term Care Homes - A Pilot Study

Spasticity is an involuntary muscle overactivity which commonly follows a stroke and presents in a variety of ways. The dysfunctions associated with spasticity interfere with resident quality of life and adds to caregiver burden. An effective Spasticity Management Program will promote a positive change for the long-term care home sector.

E-POSTER #3 - Enhancing Food in Long Term Care: Phase 1 Identifying Enrichment Opportunities

This session will provide learnings from the first phase of a three phase project focused on improving the nutrient quality of food provided in long term care. Many older adults consume inadequate vitamins and minerals and protein. Enriching food offerings is one way to improve this situation.

E-POSTER #5 - Towards a Life Course Approach to the Organization & Delivery of Dementia Care: The Preferences, Strengths, and Need-based (PSN) Model of Dementia Care

Facilitators will provide a detailed description of the PSN Model and engage participants in discussions about care interventions align with the PSN Model. Participants will apply their learning through interactive group activities. Discussions will inform understanding of dementia care delivery in LTC and roles of various clinical and non-clinical health care staff.

E-POSTER #6 - Building a Culture of Quality - Quality Day at Kensington Health

This is the story of how one organization mobilized care providing staff to come together to learn more about quality through being part of a staff group to plan a Quality Day celebration for eight entities that make up the Kensington Health organization. The result was a surprise to everyone!

E-POSTER #7 - Fracture Prevention: A New Tool for Fracture Risk Assessment and Management in Long-Term Care

The Fracture Risk Scale is a standardized tool for identifying fracture risk for LTC residents. This poster will review the guidelines for fracture prevention in LTC as applied to the FRS, using case studies to illustrate best approaches to managing LTC residents at varying levels of risk for fracture.
